If You Need to Refer A Patient, We Are Here To Help
Do you have a patient who could benefit from being seen by a pediatric GI specialist? If so, please fax a referral to Pediatric Gastroenterology of Idaho at (208) 528-1939 or call (208) 528-1039.
We will take care of the rest. Once we receive your referral request, we will reach out to your patient’s family to schedule an initial consultation. Pediatric Gastroenterology of Idaho will make sure you stay up to date on your patient’s care.
In your referral, please include the following details for our team:
- Patient’s name and date of birth
- Reason for referral (Diagnosis)
- Applicable Chart Notes
- Applicable Labs/Imaging
- Growth Charts
- Parent or guardian’s contact information
- Current insurance card
